Hi,

Trying to figure out what the difference is between TRT with an audiologist or pink noise therapy through this network to treat hyperacusis. I’ve just tried 2 weeks in quiet and tolerances and ear fatigue continue to be a problem, so I think I need to go all in on H treatment now.

The first difference is cost.  The CD is inexpensive and the cost for TRT is expensive unless your insurance covers the cost.  A lot of insurance policies will not cover the cost of TRT for H. 

If you go to a certified audiologist in TRT, you get an experienced professional to guide you through the program, and provide you with the best chance to get back your sound tolerance. 

The audiologist will do full testing on your hearing capabilities and determine your loudness discomfort level. They can also test your tinnitus and how it interacts with our H.  Your ears  will also be fitted with sound generators that work best for you. They may offer you different types of pink/brown sound that you can tolerate.  

Sound generators worn in the ear are thought to be more effective in improving sound tolerance than lessening to a CD.

The pink noise from the network CD/flashdrive and the pink noise from the sound generators are the same.  It is important to understand that.  The difference lies in the delivery system.  With the network pink noise you deliver the sound to your ears through headphones.  With TRT pink noise is delivered through hearing aids.  Hearing aids offer you total mobility because they are battery operated and less cumbersome than headphones.  That advantage comes at a cost.  Working through a trained clinician to diagnose, treat, and provide on going counseling throughout TRT is the very best method.  However, if cost prevents you from obtaining TRT then the network pink noise is the next best alternative.  Be clear on this point however, the pink noise is the same with both but the delivery system is different.

Do you think your pink noise via headphones can work for a slow onset / worsening H?

Here's more about my history:

If I look back at my history, I think my tolerances started slowly dropping a few years ago as I always felt concerts were a bit louder than others (and I did wear ear plugs). Then after the Cipro in Nov 2017 when my ears started feeling tight and fluttery, I recall wearing ear plugs at a conference keynote session in March 2018 (and no one else did). Definitely a few of these situations over the next year. Then after the concert Oct 2019, the H became very obvious and LDLs dropped to low 70s. Then after the arcade in Feb 2020, they dropped to where they are now - prob 60s. I’m wondering if anyone with a history like this has responded to TRT or the surgery?

Yes, absolutely.  Decreased sound tolerances many times are cumulative over the years.  In other words, it can start out slow and barely noticeable and degrade (worsen) over time to the point where you wonder what happened to get me to this point.  Figuring it out and tying it to one event is usually impossible and often a waste of time.  However, it is in our nature to try to make sense of it.  It sounds like you are doing all the right things as far as protecting your ears in loud surroundings.  Now it's time to work at building your sound tolerances.  During sound therapy I kept telling myself that it took me this long (years) to get to this point, now it's going to take some time to build them up again.  I am very confident that you will do well.

Thanks. Did you experience any uptick in pain as you went through sound therapy? How long did it take you to regain tolerance? Did you continue working and living normal life as you did sound therapy?

Rehab of any kind has it's bumps but you can't lose sight of the goal.  There will be days when you think this is making me worse.  I had so much pain in my ears at times I thought that either  this will make me better or I will go deaf.  But...eventually after almost 7 months I noticed that the pop machine as I entered work was not as loud anymore.  I was jubilant.  Often I have said that many times people throw in the towel when they were on the brink of successfully improving their hyperacusis.  It works but you have to be motivated to see it through to the end...then you will move closer and closer to the mainstream of life.  You can do this!

What does it matter if u have a nerve 8 dammage of the part on the hearing or a central dammage state couse of acoustic truma?the pink noise trt could not help this hard case in those 2 states of hearing aids !trt can only make u dont hear the t sounds..that all,only if u dont higher the volume.some patients higher the volume and worst the h tolerance and the t volume or the t frequency....so what u are dealing could not be good for nerve or brain hyperacusis dammage...it for damage in the stapes maybe..nothing else! U can drop buttocks to the first muscle near the outer ear if it one side of hypercusis that maid by acustic noise disorder..that's what I readed here.its not my case.. Unfortunately, it seems irreversible ... By the way, the Trt didn't hurt me but didn't help at all ... My t is two-sided and travels a lot between the two ears and some years its stuck to one side to set. Higher frequency t is always heard more than before ,much higher frequency than the first side it was at the 1 week from the a t.
and is usually a much more objective objective pulsatile than heart rate and not as subjective as before.i did the trt only in quite stages ..not outside,
And not where there is high frequencies .u must understand it's only if u live alone and 100m from other humen or other objects or animal that make sounds,in home or buildings!..else its those not worth to try..if u do not have nerve or central dammage...I have shock states!then immitaly after t- there is the harmful ever pain in the 2 ears with no man can tolerate ..u want to duy those seconds..I want be in coma alot of times...I cant dill with it..no help...from no ent or nirologs....I dont have money to put on biofeedback or yoga that did not help anyone in my state! I dont wanna be in pain when I need to be in the busses or in the streets or in any place..all the frequncey ranges qulepsed! trt is no good for me..and a lot of us.no pink no red no white can help when there is such dammage...and the world do nothing to help Us. I have also tmj pain and kliks coused by the t or h in some years after the acustic shock...Meniere's disease does not come close to the pain level of pain-type hyperacosis. May the lord help us!!!!.no humen bing can function in such state for so long years! The World Health Organization has been a crime to us for many years.Because it's not like a cancer that kills..it does not put anyone in a wheelchair..so no one feels or see what we experience.No account at all for our horrible injury.It is more horrible than full deaf.He does not suffer pain and not stop from moving in any noise situation. We do not deal with lightweight possessions of noises or sounds that do no harm at all to a healthy person. The horrible level of hearing damage is not addressed The stress and aggravation of hearing level and the most difficult day-to-day functions, if any or all, can be performed under and after the pain. We are not treated in terms of relief, home help, communication that is almost non-existent with no noise recognition of our auditory disability. stronlg. I hate a lot of ents and niroligs..no outonyrolog Andersend that ct scan wont help to find where is the hearing dammage couse its function and not fractures issues that ct can maybe see. Meg scan for the all hearing system -that what I seek .they do not do in my state at all ..the medical hospital dont knew The data that needs to be put into the meg device to be able to diagnose the level of nerve or brain damage or its part.shame and discharge for them .

What surgery did u made?
For what?
Ye if there is a nerve 8 or central h dammage, the tolerance of the hearing system will qulepse from year to year..or less ..The more you hear the louder and louder for a longer duration of sub-seconds, which hurts you.couse of it u must musk with breaks. in the long time other ear u can hear even another frequency ... can be objective t on one side and pulsative on the other ...its real hard issues to dill...I must be in quite places without the noise that hurm me that can be appeared .were is the central frequencies that u can not tolerate?the high?the ultra high I ment morez then 8000hz,the mid(500 till 2000hz), or the low(til 0 - 500hz)? Its real need be ask ....

nerve dammage it mean affernt or eferent nerve cell,that leading the noises ir sound as nerve signal inside the ear,from the nearly and of the coclea,til the primery hearing center,u gave 2 of those,each side cicontrol the other ear,and also yhe ear that located down to him .
the type of those cells became diffrent when u go on the nerve root beetwin the the nort primiary hearing centers,
