Hi Catalanear,

I think something like TRT is a reasonable first step, especially if you do not get long lasting setbacks from sound. In my opinion, surgery shouldn't be considered until you've exhausted other approaches. This includes desensitization techniques like TRT and if that doesn't work possibly trying medication. I'd just be sure to do things gradually and only go at a pace you are comfortable with. There is a paper about using noise generators to resolve myoclonus in 2/4 patients.

East CA, Hazell JW. The suppression of palatal (or intra- tympanic) myoclonus by tinnitus masking devices: a preliminary report. J Laryngol Otol. 1987;101:1230-1234.

I would imagine TRT could be better since it also attempts to address psychogenic factors that surely can make things worse. With regards to tensor tympani flutter, Jastreboff claims treatment of "misophonia" results in its elimination. This seems to oversimplify the issue, but I would expect their misophonia treatment strategies to be helpful.
