Hi,

I had a nervous system breakdown due to factors unrelated to sound. Since then, i have these mini nervous breakdowns due to sounds i normally did not have a problem with. But i never notice it right away. It wells up after. like my nervous system gets too agitated and then crashes. For example: i play guitar and seem to be okay, but feel a bit nervous after playing a while. Then a couple of hours later i feel fatigued and start to notice i can not handle the sound of a youtube video anymore. The next day the startle reflex i have to guitar and youtube videos is x100 and i have to set the volume extremely low. This seems not to be hyperacusis, or not severe. There is no pain. There is just an extreme nervous system response.

How do i treat this other than exposure therapy?

Hyperacusis does cause an exaggerated startle response.  Hyperacusis does not always cause pain (mine does, for many people it does not).  With sounds are perceived as being "louder" than they really are.  There are different degrees of hyperacusis.

Gentle sound exposure is the primary treatment for hyperacusis; it typically helps, but your results may vary.  Go to an audiologist that treats hyperacusis and see you you actually have it.  I don't know if you do or not.

Do you feel the pain right away as you hear the sound? What kind of a pain is it?
